Title Examining the Differential Impact of the Hidden Curriculum Among Physician Faculty and Medical Learners
Background/Purpose Although manifestations of the hidden curriculum (HC) phenomenon have the potential to reinforce or undermine the values of an institution, few studies have comprehensively measured its scope and impact. We explored the HC and assessed whether its impact varies significantly among physician faculty, residents, and medical students.
Methods We surveyed medical learners and faculty (n=470) in 2020 and measured their experience with the HC including their perceptions of respect and disrespect for different medical disciplines, personal actions, efficacy, impact of the HC, and their institutional perceptions. We used an exploratory factor analysis and regression analysis to examine the HC and its impact.
Results We discovered HC constructs reflecting negative expressions, positive impacts and expressions, negative impacts, personal actions, and positive institutional perceptions of the HC. Compared to residents, faculty were less likely to report negative impacts of the HC (b = -0.43, p < 0.05) or take certain personal actions as a result of experiencing a negative impact of the HC (b = -0.28, p < 0.05). Medical students were more likely to report they were negatively impacted by the HC (b = 0.47, p < 0.05). Females (b = 0.42, p < 0.05) and gender nonbinary respondents (b = 0.81, p < 0.05) experienced higher likelihoods of negative expressions of the HC compared to males.
Discussion Unique dimensions and contexts of the HC exist than have been previously documented. Subgroups differ significantly in their experience of the HC. We will also present respondents' views about why the HC exists and potential solutions.
