Drawing on field experience and operational evidence from the Democratic Republic of Congo, South Sudan, Liberia, and Nigeria, this work presents the Field-Anchored Vaccine Specification Matrix (FAVSM), a practical framework that translates field constraints into vaccine product design requirements. The framework examines four fragility domains: conflict settings, displacement settings, rural-remote areas, and urban informal settlements. It is presented in a published peer-reviewed book chapter in Vaccine Development: Lessons Learned and Future Trends (IntechOpen, 2026).

The FAVSM shows that vaccine product attributes such as single-dose presentation, elimination of reconstitution steps, simplified administration, and improved thermostability are critical for sustaining vaccination continuity in fragile settings. The framework also demonstrates that the relative importance of these attributes varies by context. In high-pressure delivery environments, ease of administration may outweigh extended shelf-life when overstretched or surge staff are required to vaccinate under difficult field conditions.

Strengthening global vaccination efforts in fragile settings requires moving beyond a narrow focus on service delivery to include vaccine product design as a core determinant of performance. Integrating operational realities into vaccine research and development can improve continuity of care, increase delivery feasibility, and support more equitable immunization outcomes for populations living in high-risk, resource-constrained, and mobile settings. |
