| Methodology/Approach | Primary research, systematic reviews, and reports implementing HEARTS fully or partially, published in English up to January 3, 2026. Following the Population, Concept, and Context (PCC) framework, searches were conducted in PubMed, OSF, PROSPERO, ClinicalTrials.gov, and grey sources including Consensus AI and Google Scholar. Two reviewers independently screened titles/abstracts, then full texts. Data were charted using Covidence and the JBI Manual for Evidence Synthesis. Reporting followed PRISMA-ScR. Findings were organized into themes of barriers, facilitators, strategies, implementation, and client outcomes, aligned with the CFIR framework. |
| Results | Results: Of 3,111 studies screened, 107 underwent full-text review, and 51 were included. Twenty-three (45.1%) were from Latin America/Caribbean, 11 (21.6%) from Sub-Saharan Africa. Most (76%) focused on hypertension; three integrated hypertension and diabetes; none addressed cervical cancer. Participants included patients, health facilities, and implementers. Forty-five studies adopted HEARTS components, emphasizing protocol-based treatment, risk assessment, monitoring, and team-based care. Identified were 21 facilitators, 23 strategies, and 22 barriers. Only three studies reported fidelity, and one assessed client-level outcomes. |
| Discussion/Conclusion | HEARTS implementation showed variable uptake across components, with notable facilitators, strategies, and barriers. Further research is needed to design contextual interventions that overcome barriers and leverage facilitators. |
