Issue/Objective Despite the availability of health operational assistance funds in Indonesia, discrepancies between high financial absorption and unmet minimum service standard targets necessitate systemic evaluation. This study explored fund implementation across four public health centers in Semarang City using the eight core functions of health financing.
Methodology/Approach A qualitative case study was conducted between May and June 2025. Following COREQ guidelines, in-depth interviews were held with 12 purposively selected facility heads, treasurers, and program managers. Data were analyzed via ATLAS.ti deductively against a health financing framework encompassing revenue collection, pooling, purchasing, resource distribution, quality, efficiency, transparency, and accountability.
Results Results showed revenue collection and distribution were tailored to local priorities, disbursed in three stages, and transferred directly to facility accounts via E-Renggar. Although financial absorption exceeded 99% in 2024, quality and efficiency indicators revealed localized variations in achieving targets like early disease detection. Transparency and accountability were maintained through multi-layered audits and digital monitoring dashboards
Discussion/Conclusion While fund implementation complies with regulations and leverages digital governance effectively, future allocations must align structurally with specific service performance gaps to optimize primary healthcare delivery.
